Everyone living with diabetes is entitled to checks and tests from their healthcare team to help them live well with diabetes. This booklet will explain the care you should be getting, if you live with type 2 and help you discuss it with your healthcare team.

 

It’s got checklists for you to tick off the appointments and tests you’ve already had and space to make notes on things you want to discuss with your healthcare team.

 

Plus, we’ve got more information on how to get help if you’re struggling with your diabetes care.

 

What diabetes care to expect if you have type 2 diabetes PDF
